Please join us for Walk to Remember 2010.  Healthy Start looks forward to this special event each year – a time to honor and remember our tiny little one’s that left us too soon.

Tuesday, October 12th, 2010 • 6:00 - 7:30 PM

St. Paul's United Methodist Church
1700 N. Meridian Road, Tallahassee, FL 32312

Refreshments and activities begin at 5:30. 

Child care for all ages provided.

Hospice will coordinate games and art projects for toddlers and older children. 
Infant care will be provided by the church and held in the church nursery.

Quilt squares will be available if you wish to dedicate one for your child.  The squares will become part of Healthy Start’s annual Remembrance Quilt. We will need your written consent for the square to become part of the quilt (Please download the form below if you wish to do this ahead of time).

If you wish to have your child’s name read aloud during the service, please download the attached permission form, complete and fax back.
